NHS Confirm your address
NHS England, supported by PCSE, may send messages to patients to confirm their home address. This is part of their regular list maintenance activities.
These messages may be sent by letter, email and text message. Emails are sent from NHS Confirm your address <nhs.confirm.your.address@notifications.service.gov.uk>. Text messages are sent from NHSdetails.
The emails include an option for patients to change their home address directly with the NHS at: https://change-your-address.account.nhs.uk. This is a new service that’s being tested with patients who receive these emails.
As part of cyber security, authentic NHS text message does not contain any links.

What to do if your address information is not correct
- You may need to register with a new GP surgery using the following link https://www.nhs.uk/service-search/find-a-gp
- Contact your current GP surgery to update your address.
- If you have left the United Kingdom and do not intend to return within 3 months you should call your GP and ask to be removed from their patient list.
